Eligibility
- All residents of West Bengal (no income limit).
- The head of the family (preferably a woman) is the primary card holder.
Required Documents
- Aadhaar card of all family members
- Proof of residence in West Bengal
- Recent family photograph
How to Apply
- Register at Duare Sarkar camp organised in your locality.
- Provide details of all family members.
- Smart card issued and delivered by the department.
- Present card at any empanelled hospital for cashless treatment.
